JRG Athletics

To participate, athletes must submit the following forms: Physical Form, and JRG Athletic Agreement Card. Both athletes and their parents/guardians need to read the Concussion/Head Injury and Sudden Cardiac Arrest Information Sheets, and Code of Conduct details. After reviewing these documents, please sign the JRG Athletic Agreement Card and turn in all required forms to the main office. All forms can be found below.
